Our claim process

Contact NCIA's Claims Servicing Team immediately at 8853-1245 to 47. Preserve the damaged property until ocular inspection and physical inventory by Insurers’ Adjuster and/or an authorized representative.



Fill in a claim form provided by us and submit all relevant documents.



Receive settlement. Your final settlement will be based on your type of claim and the completeness and availability of all supporting documents.


Required documents

General requirements
  • Police Investigation Report and/or Incident Report
  • Photographs depicting the damages sustained
Building claim
  • A civil engineer’s or architect's detailed estimate for the restoration and/or repair of the damaged building
  • Copy of the building plan and/or sketch complete with dimensions/specifications
  • Declaration of real property
Stocks claim
  • Formal computation of claim or a detailed inventory of articles damaged or destroyed showing the cost of each and extent of loss
  • Inventory of affected and unaffected stocks as of date of loss, to include quantity and cost
  • Documents in support of the unit cost such as purchase invoices or receipts and delivery receipts
  • Copies of Income Tax Returns and audited financial statements for the last three years
  • Latest Merchandise inventory filed with the Bureau of Internal Revenue
Machinery/equipment/furniture/fixtures/fittings claim
  • Complete description/specification of the affected and unaffected items as of the date of the loss showing its make and model, usage and year acquired, specific function and replacement cost supported by purchase invoices
  • Written report/technical report as to the cause and extent of damage property and estimated value of salvage
  • Formal statement of claim supported by quotations for replacement/repair for the damages sustained if repairable

  • Incident report and/or police investigation report
  • Photographs depicting the damaged property
  • Proof of ownership of the damaged property by the Third Party Claimant
  • Detailed statement of claim supported with invoices, delivery receipts, official receipts and quotation
  • Sworn Statements of witness, if any
  • Medical certificate of injured third party
  • Hospital bills, medical receipts and doctors’ prescriptions

  • Police investigation report and/or incident report
  • Affidavit of witness, if any
  • Detailed Statement of Claim supported with invoices, vouchers, deposit slips and other documents to substantiate the amount of loss
  • Audit report prior to and after the loss
  • Cash count prior to and after the loss
  • Photographs of damaged premises and points of entry and exit of burglars

  • Police Investigation Report and/or incident report if loss is due to Burglary and/or Robbery
  • Detailed claim statement
  • Bill of Lading with terms and conditions on reverse side
  • Invoice and packing list
  • Waybill and delivery receipts
  • Photographs taken at the time of the loss
  • Duly stamped received copy of formal claim address to bailees, i.e. carrier or shipping company, arrastre and customs brokers/forwarders
  • Import entry and declaration
  • Container Interchange or EIR
  • Other documents which may be required by Insurers

  • Detailed statement of accountabilities of the involved employee
  • Audit report with supporting documents
  • 201 file of the involved employee with latest whereabouts
  • Termination letter of the involved employee
  • Complaint filed and/or court case against the involved employee
  • Schedule of money due to the involved employee

  • Police report
  • Notarized affidavit of the Insured’s driver
  • Certificate of Registration and covering official receipt of the insured vehicle
  • Driver’s license and covering official receipt of the Insured’s driver
  • Repair estimate
  • Pictures showing the plate number of the insured unit and the damaged portions

  • Police report
  • Notarized Affidavit of the Insured’s driver
  • Certificate of registration and covering official receipt of both the insured and third party vehicles
  • Driver’s license and covering official receipts of both the insured and third party drivers
  • Certificate of No Claim of the third party vehicle
  • Repair estimate
  • Pictures of the plate numbers and damaged portions of both the insured and third party vehicles
